If anything you asked for isn't available, we'll offer the closest alternative on the call.

For anything that feels urgent right now — significant pain, swelling, or an injury — please call us directly at (416) 369-1000 instead of waiting on a form.

Appointment availability varies week to week, so listing a couple of options in the form gives us the best chance of finding a slot that genuinely fits around your work, school, or family routine.

There's a spot on the form to let us know you're new so we can plan enough time for a thorough first visit.

Let us know your coverage situation on the form — including if you're not sure — and we'll walk you through the next steps by phone. If you don't have coverage, we'll still work with you to plan a visit that fits your needs.
