It's not required, but arriving with paperwork done makes check-in quicker. If intake forms are available online they'll be linked from this page; otherwise, arrive about ten minutes early and complete them at the front desk, where our team can help with anything unclear — in the language you're most comfortable with.

The basics a dental office needs to treat you safely: your contact details, medical history, current medications and allergies, past dental work, and your insurance or CDCP information if you have it. None of it is collected by this website — your answers go directly to our office systems.

Jot down any medications you take, relevant medical history and allergies, and bring your insurance or CDCP details. If you've had X-rays recently at another office, mention it at check-in — we may be able to request them instead of taking new ones. Even a few notes on paper speeds things up considerably.

We treat everything you send us as confidential and use it only to arrange and provide your care — scheduling, reviewing your history, or sorting out insurance and CDCP details.
